1. start by downloading the APPLICATION FORM (PDF) enclosed and save it on your computer.
2. fill in your information
3. either email it to: the Treasurer, Mrs Inês Ferreira (e-mail: ines.ferreira@incm.pt) with a copy to the webmaster, Mrs Marie-Astrid Pelsdonk (e-mail: webmaster@fidem-medals.org)
4. or print it out as a paper copy and send it by normal mail to:
